Hip pain does not always mean hip replacement. Many hip problems can be managed with medicines, physiotherapy, activity modification, injections or lifestyle changes.

Hip replacement becomes a serious option when the joint damage is advanced and non-surgical treatments no longer give acceptable relief. AAOS notes that hip replacement may be considered when medications, activity changes and walking supports do not adequately help symptoms.

The Main Sign: Pain Is Controlling Your Life

Hip replacement is considered when hip pain starts controlling daily life instead of the patient controlling the pain.

Common signs include:

  • Pain while walking
  • Pain while climbing stairs
  • Difficulty getting in and out of a car
  • Difficulty wearing socks or shoes
  • Limping
  • Pain at night
  • Pain even after medicines
  • Reduced walking distance
  • Stiffness while sitting or standing
  • Loss of independence

The pain is often felt in the groin, front of the thigh or sometimes the knee.

Hip Replacement for Arthritis

Osteoarthritis is one of the common reasons for hip replacement. In advanced arthritis, the smooth cartilage covering the joint wears down. The bones rub against each other, causing pain and stiffness.

Hip replacement may be considered when:

  • X-ray shows severe joint space loss
  • Pain persists despite treatment
  • Movement is restricted
  • Sleep is affected
  • Daily activity becomes difficult

Hip Replacement for AVN

In AVN, the blood supply to the femoral head is reduced. If diagnosed early, joint-preserving treatment may be possible in selected patients. Once the femoral head collapses, hip replacement often becomes the more reliable option.

AVN patients are often younger than typical arthritis patients, so implant choice and long-term planning become important.

Hip Replacement After Fracture

In elderly patients, certain hip fractures cannot be reliably fixed with plates or screws. In such cases, partial or total hip replacement may be recommended to allow earlier movement and reduce bed-related complications.

This is usually decided based on age, fracture type, bone quality and pre-injury activity level.

When Hip Replacement May Not Be Needed Yet

Hip replacement may not be needed immediately if:

  • Pain is occasional
  • X-ray changes are mild
  • Pain improves with rest
  • Physiotherapy is helping
  • Medicines are needed rarely
  • Walking distance is still good
  • There is no major stiffness

In such cases, the surgeon may suggest monitoring and conservative care.

When You Should Not Keep Waiting

You should not keep delaying evaluation if:

  • You are limping daily
  • Pain wakes you from sleep
  • You cannot walk comfortably
  • You need regular painkillers
  • Hip stiffness is worsening
  • You are avoiding social activity
  • You are dependent on family for basic tasks

This is usually decided based on age, fracture type, bone quality and pre-injury activity level.

What Tests Help Decide?

A hip evaluation may include:

  • Clinical examination
  • X-ray of pelvis and hip
  • MRI if AVN is suspected
  • Blood tests if infection or inflammatory disease is suspected
  • Fitness evaluation before surgery

The decision should never be based on X-ray alone. Symptoms and patient goals matter.
